Abstract

Translational bioengineering bridges laboratory discoveries with clinical products through a sequence of engineering, regulatory, and commercial milestones that most academic innovations never complete, yet the factors that distinguish programmes that cross the translational divide from those that stall remain poorly quantified. We evaluated 226 translational bioengineering programmes conducted across centres in Italy and Switzerland between 2014 and 2024, spanning five technology sectors: implantable medical devices, diagnostic biosensors, drug delivery systems, tissue-engineered products, and computational health tools. A Translational Bioengineering Innovation Index (TBII) was constructed from five sub-scores -- technical readiness level, regulatory pathway clarity, clinical evidence strength, manufacturing readiness, and commercial viability assessment -- with weights from regression against achievement of a defined translational milestone. TBII correlated with milestone achievement at r = +0.84 and discriminated successful from stalled programmes with an AUC of 0.884. Implantable device programmes scored highest (mean TBII 0.824), while computational health tools trailed at 0.602. Only 35.8 percent of programmes exceeded the 0.75 threshold. Technical readiness carried the largest regression weight (beta = +0.276), followed by regulatory pathway clarity (beta = +0.232).
