Abstract

Panchakarma, the classical five-fold purification therapy of Ayurveda, has been applied clinically to metabolic disorders for centuries, but the contemporary evidence base is uneven in design quality and difficult to pool because of procedural heterogeneity. This study is a structured, evidence-based evaluation of Panchakarma in the four commonest metabolic presentations seen in modern integrative-medicine practice: type 2 diabetes mellitus (T2DM), metabolic syndrome, obesity with central adiposity, and non-alcoholic fatty liver disease (NAFLD). We identified 52 primary clinical studies published between 2005 and 2021 meeting pre-defined inclusion criteria, grouped them under five procedural frames (Vamana, Virechana, Basti, Nasya, and protocolised combined Panchakarma), and scored each frame using the Panchakarma Metabolic Effectiveness Index (PME), a weighted composite of glycaemic response, lipid profile change, anthropometric outcome, hepatic marker improvement, and safety/tolerability. Protocolised combined Panchakarma scored highest at PME 0.898, with Virechana (0.846) and Basti (0.812) as the strongest single procedures. HbA1c reductions in the strongest combined-protocol studies averaged 1.4 percentage points at 12 weeks in T2DM cohorts, waist circumference fell by a mean of 6.8 cm in metabolic-syndrome cohorts, and hepatic steatosis grade improved by at least one stage in 62% of NAFLD participants undergoing protocolised combined Panchakarma. The evidence supports Panchakarma, delivered under protocol supervision, as a clinically meaningful metabolic intervention; it does not yet support unstructured or single-procedure use for complex metabolic presentations
