Guduchi (Tinospora cordifolia) is the most extensively characterised immunomodulatory herb in the classical Ayurvedic pharmacopoeia, with therapeutic use spanning infectious disease recovery, chronic inflammatory conditions, and general immune support. The classical rasayana designation (rajanyadi / madhyama rasayana) explicitly identifies Guduchi as supporting appropriate immune function rather than simple immune stimulation, which is the more nuanced mechanistic claim that modern immunological research has progressively confirmed. Over the past two decades, molecular characterisation has identified the principal bioactive compound classes (alkaloids including berberine, diterpenoid furanolactones including columbin, arabinogalactan polysaccharides) and the principal immunological effector pathways (macrophage activation, NK cell activity modulation, cytokine balance modulation, specific T-cell response modulation). This study reviews 48 primary preclinical and clinical studies published between 2005 and 2021 evaluating immunomodulatory effects of Guduchi, grouped under five effect frames: G1 (innate immune modulation), G2 (adaptive immune modulation), G3 (cytokine profile modulation), G4 (post-infectious immune support), and G5 (integrated clinical immunomodulation in chronic conditions). Each frame was scored with the Guduchi Immunomodulatory Index (GIMI). Post-infectious immune support (G4) scored highest at GIMI 0.826, followed by cytokine profile modulation (G3) at 0.800 and integrated clinical (G5) at 0.782Downloads
