Abstract

Systems medicine - the multi-scale, network-oriented approach to health and disease that grew out of systems biology in the 2000s - turns out to be an unexpectedly natural fit for traditional healthcare systems. Ayurveda, Traditional Chinese Medicine (TCM), and other traditional systems have always been network and multi-scale in character: patient-level constitutional patterns, multi-target formulations, and environmental integration have been part of their conceptual vocabulary for centuries. What systems medicine adds is the methodological apparatus to translate these traditional concepts into quantitatively testable models. We consolidated 76 peer-reviewed studies published between 2012 and 2024 that explicitly applied systems-medicine methodology to traditional-healthcare contexts. Studies were grouped into five integration frames: S1 (omics and systems-biology integration), S2 (network pharmacology of traditional formulations), S3 (P4-medicine alignment with traditional frameworks), S4 (multi-scale molecular-to-clinical modelling), and S5 (clinical-phenotype systems characterisation). Each frame was scored on the Systems Medicine Integration Index (SMII), a weighted composite across conceptual alignment, methodological rigour, empirical validation, translational readiness, and interoperability. Network pharmacology of traditional formulations (S2) and clinical-phenotype characterisation (S5) came out on top at SMII 0.876 and 0.848. Our reading is that systems medicine provides the most natural bridge between traditional and conventional frameworks and should be prioritised in future integrative-medicine research infrastructure.
